Add --dp_balancer cache_aware using bounded token-prefix dispatch history and load-aware fallback. Keep request groups together, derive hash granularity from existing cache configuration, and preserve the default balancer.
This improves the chance of DP-local prefix reuse without sending all repeated prompts to an overloaded rank. Routing hints do not assert actual KV residency; disabled-cache requests bypass affinity, and temporary prompt mappings are released by their owner.
Tests: configured granularity, affinity, overload fallback, LRU eviction and mapping ownership (13 passed).
